Ownership, PIC & Pharmacy Changes

Educational guidance for organizing ownership, pharmacist-in-charge, address, and other pharmacy change requirements.

Educational information: Requirements can change and may vary by pharmacy type, ownership, service, payer, and jurisdiction. Confirm current requirements with official agencies and qualified professionals.

Common change events

Events such as a change in ownership, pharmacist-in-charge, pharmacy name, address, or location may trigger board, DEA, payer, NPI/NCPDP, wholesaler, and other update requirements.

Coordinate across systems

A pharmacy change should be managed as a cross-functional project. One update may affect licensing, controlled-substance registration, credentialing, payer contracts, banking, tax records, wholesalers, and claims systems.

Use a change tracker

APEX recommends maintaining a dated tracker listing each agency or business partner, the required form or notice, submission date, approval/effective date, and supporting documentation.
